Insurance and Medi-Cal

Step One: 

Send BLC a copy of your insurance cards and a copy of psychological evaluation will be obtained stating:

Step Two: 

BLC will submit a verification of benefits, once benefits have been verified a payment contract will be sent to individuals who have a patient responsibility.

Step Three: 

BLC will submit an authorization request to the insurance company for an FBA. An authorization will be received within 10 business days and an assessor will contact parent/guardian.

Step Four: 

The FBA will be completed within 8 weeks. Final treatment plan will be reviewed with parent/guardian and submitted for treatment authorization.

Step Five: 

Supervisor will contact parent/guardian and services will begin.

To begin the process of obtaining your behavioral health treatment through your insurance company, complete and return the attached documents to BLC for processing.

  • Print form and fill out to the best of your ability (please see link below)
  • Copy of medical card (front and back)
  • Diagnostic evaluation stating diagnosis of autism or Autism Spectrum Disorder (ASD)
  • Send Intake From along with additional documentation to:
